Preadmission information for Clatsop Care Center

 

Thank you for considering our facility to provide the services you require. We hope to make your transition as smooth as possible, and with that thought in mind, we ask your consideration in following the four steps listed below.
1. Select a Local Physician
All persons admitted must be accompanied by Physician's Orders which provide us with the necessary orders and information to provide care and treatment. The physician must be local and able to oversee the medical treatment during the person's entire stay.
2. Make Arrangements
The arrangements required are the completion of admission forms, making financial arrangements, providing insurance information and designation of a person to function as "Responsible Party" (if you are unable to function for yourself). In some cases, information may be given by telephone and the necessary paperwork signed upon admission.
3. Select Clothes and Personal Items
We encourage our residents to be up and about as much as possible, therefore, daytime attire is preferred. The amount of clothing required for each resident may vary, but please plan for several changes. We recommend clothing that will be easy to manage and that will withstand the laundry treatment required in a care facility such as ours. Footwear should be chosen with comfort and secure footing in mind. Please make sure that all clothing items are marked for identification, or inform the nurse that the clothing needs to be marked. It is important to permanently mark all items brought to the nursing home, particularly clothing, in order to facilitate the garments being returned to the proper resident.

WHAT TO BRING THE DAY OF ADMISSION

  1. To insure accurate billing to appropriate payors, we will need a copy of your HMO or Medicare Card.

  2. Bring copies of all other Insurance Cards as well and your Social Security Card.

  3. A copy of Living Will/Advance Directive or Durable Power of Attorney is to be incorporated into the medical record.

  4. Our Admission Agreement will need to be signed prior to, or the day of admission.

  5. All clothing should be clearly labeled with permanent ink or labels and brought in upon admission. If the family intends to do laundry, an appropriate receptacle must be provided (laundry hamper). It would be helpful to also label any personal furniture or other items.

  6. Please check with the Admission Department to be sure we have all the proper information we will need for our records. Complete and accurate information upon admission will improve the overall quality of the stay at our facility.
